Payment And Cancellation Agreement

By submitting this form, the above mentioned company hereby agrees enters into contractual agreement with SITECH, Inc. to confer payment of $199 per first attendee, and $99 per additional attendee (up to two more attendees). Special scholarships and considerations are allowed provided they are supplied in writing before the first class session. Before enrollment can be finalized, SITECH must receive full payment or a qualified company purchase order number. Cancellations will only be accepted until 48 hours prior to the first class session, at which point full refunds will be awarded. Within 48 hours, cancellations will only be refunded as SITECH credit for future purchases. No-shows forfeit all registration fees and applicable charges.
